Protein Energy Wasting in CKD Patients

A study was conducted to assess the nutritional status of CKD patients visiting Dr. B. R. A. Memorial hospital and Pt. J. N. M. medical College Raipur c.g. .A total of 50 subject from both sexes having means age 49.61 15.28 years undergoing hem dialysis were enrolled for the study. Information regarding disease history and associates co morbidities were collected from patients files. nutritional status was assessed by anthropometric measurements using body mass index BMI .the biochemical results including kidney profile , serum electrolytes and hemoglobin were recorded from the patients file. Dietary data was obtained by using 24 hour dietary recall and food frequency questionnaire. the results indicated that age, familial predisposition, hypertension, diabetes and heart disease increased the risk for renal failure. Protein energy wasting PEW is common in patients with chronic kidney disease CKD . PEW is one of the strongest predictors of mortality in patients with CKD. The International Society of Renal Nutrition and Metabolism ISRNM expert panel has defined PEW as a, "state of decreased body stores of protein and energy fuels body protein and fat masses . The ISRNM panel has also proposed diagnostic criteria of PEW with four categories. Cachexia is a severe form of PEW. The proposed causes of PEW are multi factorial and include nutritional and non nutritional mechanisms. The literature indicates that PEW can be mitigated or corrected with an appropriate diet and enteral nutritional support that targets dietary protein intake. Dietary requirements and enteral nutritional support must also be considered in patients with CKD and diabetes mellitus and in children with CKD, in addition to dialysis patients. Features of ideal dietary supplement have also been discussed. Dietary interventions such as enteral feeding with high protein meals or supplements might improve the nutritional status and outcomes in dialysis patients. Protein Energy Malnutrition in CKD Patients

Elevated protein catabolism and protein malnutrition are common in patients with chronic kidney disease CKD and end stage renal disease ESRD . The underlying etiology includes, but is not limited to, metabolic acidosis intestinal dysbiosis systemic inflammation with activation of complements, endothelin 1 and renin angiotensin aldosterone RAAS axis anabolic hormone resistance energy expenditure elevation and uremic toxin accumulation. All of these derangements can further worsen kidney function, leading to poor patient outcomes. Many of these CKD related derangements can be prevented and substantially reversed, representing an area of great potential to improve CKD and ESRD care. This review integrates known information and recent advances in the area of protein nutrition and malnutrition in CKD and ESRD. Management recommendations are summarized. Electrolyte Imbalance in CKD Patients

The primary function of the kidney is to maintain fluid and electrolyte homeostasis. Each day the kidney must excrete 1500 ml of water and any excess ingested sodium, potassium, magnesium and phosphate. The kidney also plays a key role in calcium homeostasis. Of the total number of patients in intensive care 325 will develop acute renal failure and patients with chronic renal disease will frequently present for surgery. The treatments for renal dysfunction may themselves cause disturbance in fluid and electrolyte homeostasis, particularly the use of diuretics and renal replacement therapy. Loss of normal renal function may lead to major changes in fluid and sodium balance. Volume status assessment will be required in patients with renal dysfunction because they are at risk of both hypovolaemia and hypervolaemia. Loss of renal function will lead to retention of potassium and magnesium with serious systemic and cardiovascular consequences including cardio respiratory compromise and collapse and cardiac dysrhythmias. Hypocalcaemia will result from reduced renal calcitriol production and phosphate retention. The identification of fluid and electrolyte imbalances and the management of clinically significant changes are required when treating patients with renal dysfunction on an intensive care unit or who present for anaesthesia.
