  3. HUD-HHS Collaboration Three Interagency Workgroups: • Community Living Workgroup • Homelessness Workgroup • Livable Homes and Communities Workgroup

  4. Community Living Workgroup Subgroups Formed: • Capacity Building to Improve Services and Access to Affordable Housing • Affordable Housing with Supportive Services for Older Adults • Improving Coordination on Olmstead Decision • Reducing the Barriers to Community Living for Individuals Transitioning from Institutional Living • “No Wrong Door” and Universal Assessments • Expanding Workforce Development

Capacity Building to Improve Services and Access to Affordable Housing • Contract awarded by CMS in June 2010 • HUD will co-lead with CMS • HHS HUD CLI Workgroup will act as the Interagency Advisory Committee. • Goal is to address housing needs for people with disabilities and chronic conditions by: • Increasing capacity across all levels of government on how to link services and affordable housing. • Providing technical assistance and funding 5 pilot projects under MFP. • Conducting on-site team building activities between Medicaid, human service agencies and housing at specific sites • Disseminate lessons learned from pilot sites • Integrate Housing and Service Resource Coordinators into MFP demonstrations to broker collaboration across housing, Medicaid and human services

Affordable Housing with Supportive Services for Older Adults • Contract Awarded in September 2010 • Goal is to design a demonstration to: • test how housing, long-term supports and services and medical care can be coordinated to support older adults in affordable housing to: • Age-in-place in the community • Improve their health and functional status • Avoid unnecessary hospitalization and reduce healthcare expenditures • examine the use of administrative and other data to target the appropriate places and individuals for such a service delivery model

Improving Coordination on Olmstead Decision • Goal is to increase collaboration between the HUD Fairing Housing Office and HHS Office for Civil Rights (OCR) to more effectively address the housing needs of people with disabilities • HHS OCR and HUD Headquarters and Regional Offices meet regularly to exchange information regarding unmet needs that emerge through Olmstead complaint investigation • Group is now considering how information gained can inform and facilitate compliance review activities

  8. “No Wrong Door” and Universal Assessments • Goal of initial HHS Project is to maximize consumer choice through single-point of entry systems and standardized assessment tools • Issue contract to conduct environmental scan on existing models and produce “how to” manual • Next Phase – integrate the project into the HUD HHS Community Living Collaborative to: • increase integration of affordable housing information into single point of entry systems • Increase the use of universal assessment systems that are comprehensive and include housing related questions

Expanding Workforce Development Goal is to expand the Annapolis “Work Where You Live” model to additional areas to: Train individuals in subsidized housing as direct service workers (DSW) Expand the supply of direct service workers as well as the availability of personal care services in affordable housing Provide personal care to individuals in subsidized housing Provide opportunities for individuals in subsidized housing to work towards self-sufficiency 9

Reducing the Barriers to Community Living for Individuals Transitioning from Institutional Living • Develop a project for Money Follows the Person (MFP) grantees participating in the “Capacity Building” contract • Provide tangible experience utilizing new capacity building skills to access housing resources for MFP participants. • Build sustainable relationships between service & support agencies and key housing agencies in the States
