  2. Our Mission • Our mission is to discover what prevents patients from continuing their rehabilitation program, discover what we can to help each person achieve their goals set forth by the program, and maintain a healthy lifestyle after completion of the program.

  3. Windshield Survey • Our community was the Wellness Center • Located on MedTech Park, Suite 200 Johnson City, Tennessee. • Off State of Franklin Road • Located next to Franklin Woods Community Hospital

  4. Windshield Survey • Time: 6:00 am (Tuesday February 8th 2011) • Relationships during this early workout generally consist of singles. Talking with the patients it was noted this was what many of them considered “me time” to relieve stress prior to work or before starting their day. • Time: 12:00 pm (Wednesday February 9th 2011) • Around this time the gym did not seem to be very busy and active. Workers informed us that the facility tended to slow down at lunch time and pick up speed either prior to or after lunch. • Time: 8:00pm (Friday February 11th 2011) • This time was a mixture of singles and couples. Many people were interacting with their workout partner or working out alone. Some of the couples stated that by coming together it helps motivate them to come. Singles felt this was a good time to again relieve stress from work that day or just general activities from the day. *This facility had a very diverse group of member ranging from 18-80 years in age. *Many races (White, African American, Hispanic, Asian etc) were also present at these various times. *Note the Cardiac Rehabilitation program is on Monday and Wednesday at 9:30 and 1:30.

Windshield Survey Continued… • People were generally working out and socializing with one another. • Handicap Accessible: • Automatic Doors • Ramps • Several parking areas for Handicap individuals • Roads in good condition and easily accessible from State of Franklin. • Bus Route Available • Side Walks in good condition • Located in a safe area of town • Oxygen and bed located right beside of the Cardiac Rehab. Area for • emergencies. • Water fountains within short distance of workout areas • Wheelchairs available if needed • Crash cart available • Continuous monitoring -Telemetry -Blood Pressure

Windshield Survey Continued… • Restrooms easily found • Outside environment is neat and clean • Food and Drink Machines Available • No sodas • Gatorade • Juices • Only offer healthy/fit snacks and drinks • Risk of falls decreased with neat cluttered environment • Improvements/Suggestions from Patients: -Understanding of medical terms -Make education more simplified -More coping with stress classes/education -Facility Structure can make others feel on display -Patients do not like walking through exercise area in front of others during their cool down. What was missing from the community? • Very little. They are within walking distance of restaurants and Franklin Woods Community Hospital. Red Lights made it easy to access from road and maintain easy traffic flow. Bus route to aid in patients unable to drive. The community was set up to assist any type of patient and in centrally located in the Johnson City community.

Community Survey • To gain a better understanding of our community we handed out a survey and asked they filled it out with their honest response. • The survey asked the following questions of our community: • 1. What was your life like before beginning the program? • 2. What were your fears upon starting this rehab program? • 3. What issues would prevent you from attending cardiac rehab? • 4. What, if anything, drives you to attend rehab regularly? • 5. What are your three biggest fears concerning your condition and rehab? • 6. How will your life change after finishing rehab? • 7. Do you have financial capabilities to go to cardiac rehab? Yes or No? • 8. Does attending cardiac rehab add extra stress to your life? Yes or No?

  8. Our Opinions vs The Patient’s • Prior to handing out the survey’s and talking with our community we discussed with each what we thought would be the 3 main concerns among the community. • 1. Biggest concern would be having another heart attack, stroke, or whatever brought them to the program. • 2. Biggest concern with the program would be time restraints and availability to attend the program. • 3. Not continuing the program and going back into old habits after completing the program.

Survey Responses • Nine participants completed the provided survey. • 3 Females and 6 Males • Causes for attending Cardiac Rehab: MI, Open Heart Surgery, Weak Heart, and Stent Placement • Common description of life before Cardiac Rehabilitation: Stressful job, fairly sedentary, busy, tired frequently, lack of energy, and overweight

Survey Responses Continued • Fears upon starting Rehabilitation: Labeled as “unhealthy”, tolerance, time in schedule, not having enough strength, and not being able to do what was expected. • Issues preventing attendance: Meetings at work, travel, schedule conflicts, and being unable to pay. • Motivation to attend: Regular exercise, goals of weight loss, increased energy, adequate education, encouraging staff, gaining strength, and to be as healthy as before.

  11. Survey Responses Continued • Biggest fears concerning condition: having another MI, possible stroke or pacemaker placement, difficulty breathing, how to monitor individually, difficulty after rehab, and getting worse or sudden death. • Life changes after Cardiac Rehab: Regular exercise, healthier diet, continue medical follow-ups, taking better care of oneself, become more active, and lose weight. • When asked if Cardiac Rehab added extra stress to the participants life, 8 out of 9 said NO.

  12. Our Proposed Project is to Increase Education of Clients and Encourage Completion of Rehabilitation through Promotional Gifts.

  14. Proposed Promotional Gift • Upon Graduation from the Cardiac Rehabilitation at the Wellness Center, the clients will receive: • Hopeful wave of Membership Fee and the First Month’s Fee to the Wellness Center • Donated T-Shirt saying: “I Survived!”
