Total reported incidents in PC

This report analyzes reported patient safety incidents from May to December 2012 across Primary Care (PC) and Hospital Care sectors. A total of 1,436 incidents were recorded, with 876 in Primary Care (72% related to patient safety) and 560 in Hospital Care (52% related to patient safety). Key types of incidents include issues with treatment prescriptions, laboratory tests, continuity of care, and patient information confidentiality. The findings provide insight into the prevalence and nature of safety-related incidents within healthcare settings.
