  4. Obtaining Exam Worksheet • Website: www.va.gov • Place curser over “Benefits” • Select “Compensation & Pension” from Drop-down menu • New web-page, select “Benefit Information” • On alphabet list, select “E” • Under letter E, select “Disability Examination Worksheet”

  5. Overview of Exam Worksheet • Usually 5 Parts, A – E • A: Review of Medical Records • B: Medical History (Subjective Complaints) • C: Physical Examination (Objective Findings) • D: Diagnostic and Clinical Test • E: Diagnosis

  6. Purpose of Reviewing Exam Worksheet Prior to Exam • IS: To ensure Client does not minimize or omit any symptoms • IS NOT: For Client to fabricate or exaggerate any symptoms

  7. Special Notes • No one accompanying Veteran in examination • Discourage writing symptoms down • Physician CANNOT assign a rating
