No Yes • Excludes • New Years day, • Good Friday, • Easter Sunday, • Christmas day • Boxing day Contact a Funeral director (see list) Inform Death Committee representatives Cause of Death Certificate obtained from Doctor/Hospital? (Tameside Hospital Bereavement Office 0161 922 4070/1) No Medical Cause of Death certificate Yes Obtain Coroner’s permission to proceed (Contact Funeral Directors) - need Removal Notice form 104 if sending the body of the deceased abroad. 0161 476 9873). Is the body to be sent abroad? Burial Abroad Yes Out of England Certificate Burial Notice Burial in Tameside No Obtain Burial Notice.
