Community Mental Health and Wellness Coalition In November 2009, CFC hosts a meeting to discuss mental health crisis and specifically that there is nowhere to refer patients looking for MH services At the time several mental health workgroups existed (i.e. Reentry Council, MAPP mental health work group) Decision to combine workgroups and form the coalition to improve the adult system of care for mental health All of the key agencies present at the initial meeting have remained involved

Coalition Activities • Held a public forum in March 2010 to identify community mental health needs • Developed mission, vision, and working groups to support efforts of Coalition • Met with City Council and Board of Supervisors about mental health needs in our community • Hired a part-time Coalition Coordinator • Received a grant from UW Initiative for Effective Nonprofits to hire a consultant for planning • Developed a Coalition Charter to more sharply define its mission and goals
Coalition Activities • Launched Healthy Transitions, a collaboration between District 9 and Region Ten, to support individuals with serious mental illness re-entering the community on probation or parole • Secured funding from a private donor to begin development of the Coalition Services Program • Held a second community forum in March 2012 to hear community concerns about mental health • Received CACF’s Inaugural Collaboration Initiative Grant in June 2012 • Launched Coalition Services Program in September 2012 • Entered into Partnership with The Center for Nonprofit Excellence for a pilot evaluation project in June 2013
