Social Care Pathfinder Initiative: NHS England funded discovery phase • Purpose: Being able to share with and from the 3rd sector • Deliverables • • A toolkit for how to bring non-statutory partners and their information onto the Leeds Care Record • An understanding of current position of voluntary sector digital position with Seacroft LCP in relation to Health • A definition of ‘role based access’ for non-statutory organisations and an understanding of what this would involve in terms of LCR development. • A funding model which balances the need to share information digitally with the costs of developing and maintaining this capability across a wide footprint of different organisations. • The definition of what information goes into a Person Centred Plan Care Plan • Technical options for shared support plan Implementation funding decision – end October 2019

Technical Options Pilot phase ( now – end October) Upload a pdf of SAM form to LCR Prove LCR upload works Existing LCR users will be able to view SAM wording, headings etc. can be road tested If implementation funding agreed (2020 onwards) Either commission an LCR e-form OR build care plan in Black Pear Depending on option chosen, attempt to get authorisation for 3rd sector users to view/create/amend SAM plan, in selected option

Possible timescales Now – end Oct Seacroft /Woodsley complete 50 SAM forms Wording agreed LCR Upload approved and tested Nov – end Mar SAM forms rolled out citywide LCR/Black Pear decision Final requirements 3rd sector access granted Summer 2020 Technical solution built and deployed Further plans/data identified for next phase

Proof of concept - Work item mappingDirectory entry / resource > taxonomy > natural language Directory entry / resource – Mapping to: Generic taxonomy concept (e.g. SNOMED) > Anxiety Generic Taxonomy (e.g. SNOMED CT) • Thesaurus mapping • “I’m feeling anxious” • “I’m worried” • “Panic attacks” • “Stressed” • Mapping allows querying of repository based on: • SNOMED Concept or other descriptions (e.g. Helm) • Thesaurus terms/phrases • Plus - Title, description, other meta data

Future Sustainability of the Health and Care Third Sector in Leeds • build on Leeds's already strong foundations, to ensure the Third Sector and communities across Leeds Health and Care system are best placed to contribute to the changes and challenges ahead • plan for long term sustainability of the third sector as integral part of H&C system . • identify shorter term measures to fast track or incubate innovation. We have an opportunity to use financial transformation investment from West Yorkshire and Harrogate Partnership (Harnessing the Power of Communities) alongside local resources to help this shift. • It is important also to identify system leaders who are passionately committed to shepherding this work. • continue to be an exemplar, both regionally and nationally.
