  4. Brag Sheets • Form is on the website • nrotc.web.arizona.edu • Under Starter Forms • Everyone must do one • Helps in writing evals

  5. Blocks 1 & 2 • Block 1: Member’s Name • ALL CAPS • LAST NAME, FIRST NAME, MI • If no middle name put NMN • Example: NEMECEK, WILLIAM J • Block 2: Rank • Drop down menu • Select appropriate rank

  6. Blocks 3 & 4 • Block 3: Company • Drop down menu • Select appropriate company • Select platoon/squad • Block 4: Occasion for Report • Midterm • Check midterm box only • Final • Check Fall

  7. Blocks 5 & 6 • Block 5: Date of report • Format: YRMONDAY • Midterm: 08OCT24 • Final: 08NOV26 • Block 6: Type of Report • Check the “Regular” box • “Special” only used for MOM, ADOM, performance counseling

  8. Blocks 7a-8 • Block 7a: PRT/PFT • Use drop down menu to select most recent score • Date of inventory 08SEP22 • Block 7b: Ht/Wt • Height and Weight • WS-Within Standards • NS- Not Within Standards • Block 8: Sail Qualifications • MIDN should check none

  9. Blocks 9 & 10a • Block 9: Swim Qualification • Check the box that you achieved • Date of Swim Qual • Same format as all other dates • YRMONDAY • Block 10a: Duties assigned • i.e. Marching Member, Squad Leader, Company Commander, etc

  10. Blocks 10b & 11 • Block 10b: Major • Put in your major • Block 11: Honors/Awards • Any you have received this semester only • i.e. MOM, ADOM, any university award, etc

  13. Last Things • Do not print off evals until finalized • Email the evals up the chain of command • A completed sample will be hung in the Wardroom for reference • Middle of Semester • Final forms due Friday, 24 OCT • End of Semester • Final forms due Wednesday, 26 NOV
