Service Model • Wrap Around Model • stabilized the children, family in the community • service provided to individual, group and family by a team of professionals including psychiatrist, LCSW, MFT, psychologist • System of Cares • home-based programme, focus on home-living • prevent the removal of children from their home • involve case manager, parent advocate, therapist

Service Model • Contiuum of Care • base on the severity of the problem of the consumers to provide different level of care • Outpatient > Intensive Outpatient > Intensive Day Treatment > Hospital • Preventive service • develop early identification service for aged 0-5 • develop assessment tools

Practice environment • Participation of Prosumers • employ the ex-consumer to be staffs • involve the prosumers in leading the self-help group with close and intensive supervision by social workers • acknowledge their experience and knowledge • Caseload of the social workers • Caseload is much fewer than HK • School-based service 3:8 • Intensive Day Treatment 1:10+

Practice environment • Multi-discipline Collaboration • not only focus on bio-medical model • involve different professional views on the treatment plan • clear roles among the professionals (LCSW, case managers, psychiatrist) • Single point of responsibility • treatment Involves different professionals • the case managers to gather the progress and information among different professionals

Practice environment • Service Development – new ideas to Government • Agency develop new working model and propose to the County / Funding source • The funding source provide the resources to that Agency to start new projects • Funding restrictions • Funding sources allow the agency to develop project with collaborated resources
