Background to the Contract The National Diabetes Prevention Programme is a landmark contract which, by 2020, will support 190,000 people across England who are at high risk of developing Type 2 diabetes to reduce their chance of getting the disease. Commissioned at national level by NHS (England), Public Health (England) and Diabetes UK. It is is a free, voluntary ten month programme consisting of 4 weekly sessions followed by 9 monthly sessions in a local venue, in groups of up to 20 people in daytime and evening sessions. Those referred to the service will receive tailored, personalised support to reduce their risk of Type 2 diabetes including education on healthy eating and lifestyle, help to lose weight and physical exercise programmes, all of which together have been proven to reduce the risk of developing the disease. An experienced Educator leads the sessions around healthy lifestyle topics encouraging self management and empowering individuals to make successful long term healthy lifestyle behaviour change. Participants receive resources and handouts to help them on their way. Participants also have access to 2 x HbA1C tests; one mid-programme and one on completion (in addition to the one they receive prior to being referred).

Eligibility • Your patient is eligible for this service if they meet the following criteria: • Aged 18 years or over • Not currently pregnant • Have non diabetic hyperglycaemia - a HbA1c 42-47mmol/mol (6.0%-6.4%) or FPG 5.5-6.9mmol/l reading within the last 12 months • If the blood test result has occurred within 3 months of the date of referral, and lies within the acceptance criteria, the patient is immediately considered suitable to join the programme and is offered a place. • If the blood test result has taken place between 3 – 12 months of referral, an Ingeus employed HCA will undertake a new blood test. • If the blood test stated on the referral form has taken place over 12 months ago, the patient is not considered eligible for the programme and will be discharged back to the GP.

  6. Outcome Measures • NHS England assess Ingeus’ success in delivering the program by reviewing a range of outcome measures: • BMI / Weight reduction • HbA1c Reduction • WEMWBS score improvement • Participant satisfaction survey • Attendance at sessions (more than 75% attendance)

Case Study: Derek Constable Aged 80 A former television camera and lighting expert from Nottinghamshire is embracing the opportunity to reduce his risk of Type 2 diabetes by increasing the exercise he takes and being more conscious about what he eats. Derek Constable, who’s 80 and lives in Beeston, signed up to his local Healthier You: NHS Diabetes Prevention Programme after being told by his GP he was at risk of falling victim to the life-changing disease. Just four months in to his ten-month programme, the results have been quite dramatic. Derek has lost almost three stones in weight and he says he feels years younger as a result. “My resolution to make the change needed to stay healthy is strong,” he said. As well as enjoying the benefits of losing weight and being more active, Derek also wants to continue to care for his wife, who has both dementia and mobility issues.
