  3. FY05 Grant Deadline • Procurement Process must be completed by June 30, 2009 • Reimbursement Requests must be submitted by June 30, 2009

  4. Grant Extension for FY 2006 • Submit Revised Project Budget Sheets by the COB on July 15, 2009 • Reimbursement Request with invoices only are due no later than the COB on August 31, 2009 • Deobligation of Funds if revised Project Budget Sheets are NOT submitted by July 15, 2009

  5. Files, Folders, and Formats • Revised Grant Forms available on website http://www.ohsep.louisiana.gov/grants/hsgrantprgfrmdocs.htm#2005letpp FY 2008-Grant Folders must contain all required documents • Signed Grant Award Sheet • Signed Terms and Conditions • Parish Working Group-120-06 (only if changes have been made) • Project Allocation Worksheet-130-06 • Project Application-140-06-143-06 • Authorized Equipment List (AEL) https://www.rkb.us
