The Corporation for Supportive Housing (CSH) CSH helps communities create permanent housing with services to prevent and end homelessness. • Project-specific financing and expertise to help create supportive housing • Capacity building to strengthen and expand the supportive housing industry • Public Policy Reform to build an efficient system for producing and financing supportive housing

Background on Creating a Housing Options Tool • National and local conversations on streamlining the process of connecting clients with the housing options that can best meet their needs • Need emerged for a comprehensive tool to help individuals and families prioritize housing options • In an effort to address this system gap, the CSH Illinois/Indiana hub office staff began developing the framework for a housing options tool

Housing Options Tool (HOT) Objectives • Provide a complete listing of permanent housing and prevention options for consumers and case managers • Easy to use • Comprehensive • Accessible via internet • Develop a way to rank housing options to help case managers and consumers identify and target their housing interventions

Solutions We Hope to Provide • Consumers • Provide all eligible options with updated information in one place; Provide accurate referrals; Take the luck out of it • Providers • Create a central directory of housing; Create a clear way to target interventions and assist case managers in talking through appropriate options with clients • System • Provide accurate referrals; target interventions to the most appropriate; streamline referrals and time in shelter; track housing eligibility and understand our system barriers

Indiana Housing Opportunity Planner and Evaluator (IHOPE) • 1st statewide customized version in the country • Screens all users for HPRP eligibility • Builds on data from the Indiana Housing Inventory and survey of housing providers • Will include eligibility information from 200+ housing options in the state of Indiana • Includes a ranking system to help prioritize housing options • Allows registered users to view responses and results • Includes data tracking capabilities

Options in IHOPE • HPRP • Prevention • Stay Housed: Assistance stabilizes household in current environment • Stabilize: Assistance alone helps household move to more stable environment • Diversion: Without some assistance would enter shelter system or be on street • Rapid Re-Housing • Short-Term: Homeless household requires money and light follow-up services up to 6 months • Long-Term: Homeless household requires longer term assistance (6-18 months) and follow-up services • Emergency Shelter, Transitional Housing, Permanent Supportive Housing

Current IHOPE Process • Members of public can visit www.indianahousingoptions.org • Will select county from list • Contact HPRP agency provided • Registered users can login to site to complete assessment • All assessments assigned a number (not just those eligible) • Eligible assessments will list specific HPRP programs in ranked order • Good idea to note assessment number in client file • 1767 surveys completed so far (although ~400 were testing)

Future IHOPE Process • Members of the public and non-HPRP agencies will be able to complete assessments • If client has completed IHOPE before and brings record number: • Login to IHOPE, enter record number and click “Load” • You can view all client responses as well as recommended HPRP eligibility categories • If client has not completed IHOPE you can complete the tool together and then follow the steps above to view eligibility results • Clients who are not eligible for HPRP will continue through tool to permanent, transitional and emergency housing options

Future IHOPE Possibilities • One click referrals and applications • Real time unit availability • In conversations with socialserve.com • Integration with HMIS • Open and data-driven conversation about admission criteria for Indiana programs • Evaluation and Improvement • Understand data for Indiana and what we should do next as a system, but also improve IHOPE for users
