  4. Main Problems • Entries too generic & vague • No consistent progression & improvement • No reflection on targets, action plan and achievement • No capturing of key data & questions

  5. Follow Up Instructions • Complete personal particulars & affix recent • passport-sized photograph • Monitors are to e-mail template to all by today • Start completing journals. Use March holidays for • fine-tuning entries • Get endorsement from assessors once • completed. Do not wait until May. • Possible to pre-date and / or back date entries
