  6. ผลเสียของความปวดหลังผ่าตัด (ACUTE PHASE) Increase sympathetic tone • CVS • กล้ามเนื้อหัวใจใช้ O2มากขึ้น • เลือดมาเลี้ยงลดลงจากการหดตัวของหลอดเลือด • เพิ่มความเสี่ยงต่อกล้ามเนื้อหัวใจขาดเลือด

  7. ผลเสียของความปวดหลังผ่าตัด (Acute phase) Increase sympathetic tone Respiratory system • phrenic nerve จะทำงานเลวลง • ปวดแผลจึงไม่ยอมหายใจลึกๆ และไอไม่ได้แรง • ทำให้เกิดภาวะแทรกซ้อนทางระบบหายใจง่าย GI • paralytic ileus

  16. Acetaminophen • Actions : CNS by inhibitprostaglandin synthetase • For mild pain, fever • Increase effect when combined with NSAIDs • Doses: 325-1000 mg/ dose

Dexmedetomidine • highly selective alpha 2 agonist • shorter half-life than clonidine • sedation and analgesia with minimal respiratory depression • infuse 1 hour before the end of surgery and continued in PACU • Lower opioid requirements • better pain control • not associated with respiratory depression in the morbidly obese patients

Systemic drugs: weak opioids Tramadol • synthetic opioid , weak μ-agonist activity • inhibits reuptake of serotonin and norepinephrine • effective for moderate pain • Advantages : • lack of - respiratory depression - major organ toxicity - depression of gastrointestinal motility • low potential for abuse • Dose: 100mg, total 600mg/day 25

Tramadol • Common side effects(incidence of 1.6% to 6.1%) • Dizziness , drowsiness , headache • Sweating, dry mouth • Nausea, vomiting • Should be used with caution in • Patients with seizures • Increased intracranial pressure • Contraindicated in • Patientstaking monoamine oxidase inhibitors 26

  27. Systemic drugs: weak opioids Codeine • Morphine like effect (1/10 ) • Increase dose : not increase analgesia but increase side effect • Side effects: nausea, vomiting, constipation, lightheadedness, dizziness, drowsiness • Combine with paracetamol increase efficacy 27

  31. การให้ยากลุ่มOpioid... น่ากลัว!!! กลัวคนไข้หยุดหายใจ จะมีวิธีป้องกันอย่างไร? • การประเมิน pain score, sedation score • วัดอัตราการหายใจ และ O2saturation • การให้ยาแก้ปวดให้เหมาะสมกับความปวดที่ประเมินได้ • Slow titration of opioid • ประเมิน pain score และ sedation score ซ้ำหลังให้ยา 5 นาที • ถ้าผู้ป่วยหายใจช้า... ให้ตามแพทย์, เตรียมรถEmergency และยา Naloxone … (1-4 mcg/kg IV.) 31

  41. PCA (patient controlled analgesia) ข้อดี • บรรเทาอาการปวดได้ตามที่ต้องการ • ปรับตามกิจกรรมที่ทำให้เกิดการปวดได้ทันที ข้อควรระวัง • ต้องศึกษาวิธีการใช้จนผู้ป่วยเข้าใจ ข้อเสีย • เครื่องมือ : ราคาแพง, ความผิดพลาดในการตั้งเครื่อง, เครื่องขัดข้อง
