After the Tests • Room Supervisor Responsibilities: • Verify the following: • All student names are printed and gridded properly with bubbles completely marked (grid) • Test Booklet Number and Form Number • Student Demographic Information is complete • Students have signed the Terms and Conditions on the test booklets • Accommodation Code if used • Erase all stray marks • All necessary reports have been completed: • WorkKeys Roster (student names and verification) • WorkKeys Test Materials Tracking Log • WorkKeys Irregularity Report • WorkKeys Seating Diagram • Recorded test time of start, 5 minute warning, and stop times (pages 40, 45, 49 in Supervisor’s Manual) • WorkKeys Report of Accommodated Tests (WorkKeys Special Testing Reader’s Agreement and WorkKeys Special Testing Interpreter’s Agreement, if applicable) • Return all materials to test supervisor

Important Information Separate Mailings to Return • All materials must be returned to ACT immediately after testing • If needed to store materials at any time, securely store all materials, in accordance with ACT guidelines (locked cabinet within a locked room) • Materials will need to be separated by: • Scorable and completed forms • Nonscorable Documents and Unused Forms • Each package(s) will be mailed to different addresses. • Mailing 1: • Scorable and completed forms include: • Answer documents to be scored • Completed site supervisor’s header (building header and group header, if applicable) • All used/completed test administration forms • Completed WorkKeys Scoring and Reporting Order Form • All scorable documents and completed forms will be mailed to: ACT Scoring Services McCarrel Center 2727 Scott Blvd. Iowa City, IA 52243-4059

Important Information Separate Mailings to Return • Mailing 2: • Nonscorable documents and unused forms include: • Used and unused test booklets • Unused answer documents • Unused headers (building header and group header) • Unused test administration forms • WorkKeys Supervisor Manual(s) • WKEYM Pack/Return Slip • Miscellaneous items, such as CD’s, DVD’s, etc. • All nonscorable documents and unused forms will be mailed to: • WorkKeys ACT Distribution Center 2727 Scott Blvd. Iowa City, IA 52243

Packing and Shipping Forms • Scoring and Reporting Order Form • Posted on KDE WorkKeys webpage • WKEYM Pack / Return Slip • Included when you received the tests • Site Supervisor’s Header - Required • You need one copy for each box of completed answer documents • Building Header - Optional • Only needed if you tested in more than one building • Group Header - Optional • Only needed if you want score reports separated by Room Supervisor

Site Supervisor’s Header - Required • Details for completing the Site Supervisor’s Header are on page • 81 - 82 of the Supervisor’s Manual: • Refer to the WKEYM Pack/Return Slip for needed information (was included in the shipment of test materials) • Complete sections A, B, E, F and G • Clearly write and bubble each section • Sections C, D, H, and I do not apply Supervisors Manual PAGE 82

Building Header - Optional • The Building Header is not required. • If your site is testing at more than one building and you want your score reports sorted and printed for each building, complete a Building Header for each building. • Use the Test Supervisor’s name and mailing address on this form or the results will be mailed to the other building(s). • Details are on page 83 of the Supervisor’s Manual

Group Header - Optional • The Group Header is not required. • If you want your score reports sorted and printed for each group tested, complete a Group Header for each group or class. • Use the Room Supervisors name on this form, not the Test Supervisor’s name • Details are on pages 83-84 of the Supervisor’s Manual

Packing Shipment Box(Not using Building or Group Headers) If you only tested in one building or want all results to be combined, then: Place the completed Answer Documents in the bottom of the box. Place the Site (Test) Supervisors Header in next. Place all related forms in the box (Roster, Tracking Log, Irregularity Report, and Seating Diagram, etc.) Finally, put in the Scoring and Reporting Order Form • Make sure your Site ID Number is on the Scoring / Reporting form!!!

  10. Packing Shipment Box(Using Building or Group Headers) The diagram on page 85 of the Supervisor’s Manual shows how to pack tests from multiple buildings with multiple Room Supervisors. The Answer Document Folders go in first, separated by class and then by building The Group Header goes in next followed by the Building Header. This alternates until all Answer Documents are placed in the box. Next put in the Site (Test) Supervisor’s Header. All other test administration forms go in (Rosters, Tracking Log, etc. The Scoring and Reporting Form should be on top. Do NOT use the Next Business Day Scoring Notice DO NOT put unscorable tests materials in this box! They will be shipped separately.

Labeling the Box – (Mailing 1of 2)For Scorable Documents • Once you have placed the scorable documents, headers, used test administration forms, and order form into the box and sealed it: • Label each box with your return address and the institution/site code and “ACT WorkKeys Scoring” • The shipping address is: • If you are returning more than one box of completed tests, label the boxes 1 of X , 2 of X , etc. • Ship via UPS or traceable shipping method ACT Scoring Services McCarrel Center 2727 Scott Blvd. Iowa City, IA 52243-4059

Labeling the Box – (Mailing 2 of 2)For Nonscorable Test Materials • Nonscorable Documents and Unused Forms are to be returned in a separate box • Keep items grouped starting at the bottom of the box: • Unused Test Booklets • Unused Answer Documents • Unused Headers • Applied Mathematics Formula Sheets (if torn from test booklets) • Unused Order Forms • Miscellaneous Unused Forms and Items (such as CD’s, DVD’s, etc) • WorkKeys Supervisor’s Manual(s) • WKEYM Pack/Return Slip • Label the box as “Nonscorable Test Materials” • Label each box with your return address • The shipping address is: WorkKeys ACT Distribution Center 2727 Scott Blvd. Iowa City, IA 52243 • If you are returning more than one box of completed tests, label the boxes 1 of X , 2 of X , etc. • Ship via UPS or traceable shipping method WorkKeys ACT Distribution Center 2727 Scott Blvd. Iowa City, IA 52243 The diagram on page 88 of the Supervisor’s Manual shows how to pack nonscorable test materials
