Health Risk Assessment (HRA) The Business Sponsor is VA’s National Center for Health Promotion and Disease Prevention The HRA is a Commercial Off The Shelf (COTS) Web-Based Application With a Customized Front-end All the Clinical Content (Questions, Answers and Conclusions) Have Been Customized to Adhere to VA’s patient population and Clinical Guidelines

  4. System Integration – Phase I Integration with VA’s MyHealtheVet (MHV) Portal to Pre-Populate Demographic Data HRA MHV Master Patient Record Date of Birth (mm/dd/yy) Gender Patient Data (e.g. Cholesterol, Blood Pressure)
