F., 12 anni, viene per rallentamento della crescita staturale (VC 1 cm/aa) Su nostra richiestaci viene detto che da 2 anni beve 3-4 L d’acqua al giorno… Esami: Na 149 mEq/L Osmolarità plasmatica 294 mOsm/kg, Osmolarità urinaria 174 mOsm/kg Rapporto OsmU/OsmP = 0.59 (<1) = inadeguato  Diabete insipido  Diabete insipido centrale Desmopressina  Normalizzazione osmolarità Test endocrini: normale funzione ipofisi anteriore eccetto Deficit di GH al test  RM Encefalo • Ipotesi compatibili con questa immagine? ( = PARTE DIFFICILE) • Istiocitosi del peduncolo: negative RX total body, cranio e torace, eco tiroide • Ipofisite autoimmune: negativi anticorpi anti-ipofisi (negata anche celiachia) • Germinoma: negative α-fetoproteina, β-HCG e fosfatasi alcalina placentare su sangue e liquor, citologia liquorale, non altre localizzazioni • Non biopsia ma RMN e rachicentesi seriate (rischio operatorio) MORALE: IN CASO DI SCARSA CRESCITA RICORDARSI DI CHIEDERE ESPRESSAMENTE QUANTO BEVONO LA DIAGNOSI PUÒ ESSERE FACILE FINO A UN CERTO PUNTO…
