  7. September Mail If you are Medicare eligible, or if you are a Public Safety retiree, you’ll receive mail related to those programs, too. Please identify the City’s bright yellow booklet and set it aside to read cover to cover! employee.phoenix.gov/hr/retirees

Health Risk Assessment • Complete a Health Risk Assessment (HRA) during Open Enrollment to reduce your monthly premium beginning January 2013! • Biometric data is needed for the HRA: Height, Weight, Blood Pressure, Blood Glucose, Cholesterol, and Waist Circumference HRA stands for Health Risk Assessment employee.phoenix.gov/hr/retirees

  9. Why? Health Risk Assessment • What you don’t know can hurt you. Being aware of your health risks can help you take steps toward improved health and lowering your medical premium. • Your medical premium is based on the health of everyone enrolled.   10. Health Risk Assessment What is a Health Risk Assessment (HRA)? • An HRA is a set of questions you answer about your health risk factors • Demographics • Lifestyle • Health History • Current Health Data (“Biometrics”) employee.phoenix.gov/hr/retirees

  11. Health Risk Assessment This biometric data must be included when you complete your HRA: • Height and weight • Blood pressure • Cholesterol (total and HDL) • Blood glucose level • Waist circumference employee.phoenix.gov/hr/retirees

Health Risk Assessment • Biometric data can be provided by • Your doctor, or • One of the many free biometric screenings between October 15 and November 9 In late September look for a bright yellow booklet in the mail with biometric screening times and locations. Private Health Risk Assessment Your individual HRA responses will not be shared with the City. • Aggregate (large group) reports will be provided to the City • Used for planning health and wellness programs • Used for planning education campaigns • Used for comparison with similar groups employee.phoenix.gov/hr/retirees

  18. Medical Premiums for 2013 • Retiree medical coverage stays the same. • Co-pays stay the same. • Effective January 1, 2013 • Cigna rates increase 2.4% • BCBS HMO rates increase 5.3% • BCBS PPO rates increase 5.2% Pick up a 2013 Retiree Rate Sheet at the door employee.phoenix.gov/hr/retirees

  19. Can I Waive and Re-Enroll? Yes, you may re-enroll in City retiree medical coverage during Open Enrollment if you’ve had CONTINUOUS and COMPARABLE COVERAGE since the date you waived the City’s coverage. Evidence of continuous, comparable coverage is required. Please speak with a Benefits Analyst for details. employee.phoenix.gov/hr/retirees

  20. B: Yes! D: No! Medicare Part B • Timely enrollment in Medicare Part B is required if you or a covered dependent becomes eligible for Medicare for any reason. • Do not enroll in Medicare Part D! employee.phoenix.gov/hr/retirees
