  3. Health insurance is critical • Health care can be extremely expensive in the United States • For Example: • Emergency Room Visit: $1,500 • Appendectomy: $20,000 • 6 Week Stay in a Hospital: $200,000 • Unexpected accidents and illnesses do happen

  4. An Emergency Room visit… …could easily cost $1,500 or more.

  5. A ruptured appendix… …could cost $20,000.

  6. A six week hospital stay… …could cost $200,000.

UW-Madison Health Insurance Mandates • Non-immigrant visa holders and their visa dependents mustcomply with the UW-Madison insurance requirements which meet or exceed the Federal and Wisconsin State insurance requirements • UW-Madison requires that all non-immigrant visa holders and their visa dependents holders purchase SHIP or waive with qualifying insurance Notravel insurance plans or private policies will be accepted

  10. To be compliant: A. Waive SHIP • Submit a qualifying Waiver Application By February 14th with proof of insurance OR B. Purchase SHIP • Purchase SHIP for yourself and any visa dependents By February 14th with credit card, check or exact cash

  11. A. Waiving SHIP : Notravel insurance plans or private policies will be accepted • Provide proof of group coverage • WI State Health Insurance Applications signed and dated by your department • Insurance ID card • Waiver Application

  12. Automatic Waivers Must Meet the Following Criteria: • Paid position with the University of Wisconsin Madison (teaching assistantship, research assistant position, or a project assistant position) • Coverage must take effect on or before February 1st of this year

  16. After enrollment? • You will receive a SHIP membership card by mail • Keep your membership card with you at all times • Card used for medical appointments and pharmacy visits

  18. How to use SHIP: • 1. Must use University Health Services for all available primary and preventive care services • 2. “In-Network” providers are available for instances when UHS is closed or unable to provide necessary services • 3. Clickthe“Find ‘In-Network’ Providers” link on the SHIP website and contact our office if you experience any difficulties
