Current Program Information • Program name: Healthways Care Support • Program effective date: 1/1/09 • Program phone number: 1-866-710-4842 • Chronic condition management for the following conditions (for those that qualify): • Diabetes • Coronary Artery Disease and Heart Failure • Chronic Obstructive Pulmonary Disorder • Asthma • Impact Conditions: • Back pain • Osteoarthritis • Atrial fibrillation • Fibromyalgia • Acid related disorders • Irritable bowel syndrome • Inflammatory bowel disease • These programs are serviced by the same team of Healthways nurses that have been working with SCSC members since 2003.   3. Transition from the BCBS of Minnesota program • The previous program name was BluePrint for Health® care support. • The previous phone number was 888-264-1744. This number will be answered through 3/31/09 by Healthways clinicians with information about program offerings for South Central Service Cooperative members. • A transition letter was sent around 12/15/08 to previously active disease management program participants explaining the change to the program name and phone number.   4. New Program Features The Member Portal is a new customized website for delivering health-related material to SCSC membersvia the Internet that is condition specific. Additionally, the Member portal features the Healthwise Knowledgebase, including interactive tools, searchable health topics, drug references and other related information.
