Anchor Submissions Alternate Achievement Requests – Due 9/9/16 • Providers with a current approved Alternate Achievement request must submit a new request • P4P QISMC baselines that are below MPL can submit for a 12.5% gap closure in DY6 instead Stretch Activity – Due 9/30/16 • Still waiting for final menu approval from CMS • The draft menu is found in the Draft Cat 3 Operational Details Document on the Waiver website

  6. Cat 3 Corrections and Cat 3 PFPM • HHSC will notify providers who submitted Cat 3 corrections of any needed follow up or approval the week of 8/22/16. • HHSC will notify providers of any needed follow-up the week of 8/22/16 regarding the PFPM Baseline templates that were submitted.

DY6 – Waiver Extension Year Waiver Renewal Planning Timeline: • August 26, 2016 - HHSC will begin providing approval of requested MLIU reporting exceptions, required QPI/MLIU changes, and submitted next steps or request additional information. • September 30, 2016 - HHSC will approve or deny the additional information submitted in response to HHSC comments. • October 1, 2016 - Approved DY6 projects are effective.
