  3. Arrival/ Buses • Students need to know their bus #. • Check the district website (www.sayrevillek12.net) for times and stops. • Follow traffic patterns for AM drop-off. (7:45-8:10). • Parent parking has been designated for PM pick-up. Form two lanes around the island. • Homeroom begins at 8:15. Please be on time. • Students are only permitted to ride on their designated bus.

  4. Absent • Parents need to call the call out line at 732-525-5291 for absences. You will also receive a call from Instant Messenger/Powerschool. • If your son/ daughter will be out for more than 3 days, please call the guidance department to request missed assignments. Guidance Extension: _____ • An absentee note is required when students return to school. • If your child has a chronic illness, you may want to let your child’s teachers know and certainly the school nurse.

  5. A Note from the Nurses! • All medications was be brought in by a parent with doctor’s orders. No medication, even over the counter medication can be carried by a child. We must have a doctor’s  order for any medication and the parent must supply all medication. • All medications must be in original container – labeled with child’s name and dose and time of administration from pharmacy – Doctor’s order and stamp • If your child is running a temperature and/or vomiting they need to be home at least 24 hours fever free with no medication. • Parents notes are only accepted for a one day gym note. More than one day requires a doctors order. • All doctor’s notes should be brought to the nurse.

  7. ATTENDANCE • Attendance Contract – Hopefully this will help us deter poor attendance habits from forming. • History – we worked to correct problems in the HS. Many of the attendance problems stem from bad habits formed in the elementary and middle school years. (This has caused HS students to lose credits for classes/entire schedule) • Possible retention with more than 20 absences. • Continued lates will cause Administrative detentions and family involvement.

Inclement Weather - Parent link messages - district web site www.sayrevillek12.net Emergency Parent Contact * It is imperative that we have current accurate contact information. • Please remember to go on Parent Portal and update emergency contacts • If they are not updated with accurate information – students may not be allowed to attend functions (It creates a dangerous situation for the child)

  10. Lockers • Each student is given an individual locker. • Please keep it clean. Stickers are not permitted. • Each student is given a lock. Replacement fee for a new lock is $4.00 • Students should keep their combination in a secure spot and should not give their combination to anyone. • Lockers may be inspected at anytime by school officials. • Students should not set their lock to open. It is the responsibility of the student to lock their locker.

  11. 6th Grade - LOCKER USE DURING THE DAY! • You are only allowed at your locker: • Before Homeroom (Books for 1,2, & 3) • Before 4th period (Books for 4, 5 & 6) • Before Lunch (7th period) • After Lunch (Books for 8 & 9) • End of the day!

Classes Language Arts, Math, Science Physical Education/Health, Science, Social Studies and Specials. • Each teacher will provide the student with a recommended list of supplies for their class within the first few days of school. These supplies are only suggestions and are not mandatory. Recommended supply lists are also on the district website. • Students are expected to get to class on time and be prepared. • Students may carry only the “sling/string” or small tote bags in the hallways.

  14. Physical Education • Students will have physical education class everyday, except for one marking period of health. • Students are to have a change of clothes (T-shirt, shorts, sweatpants, sweatshirt) in their gym locker. Please remind them to take them home often to wash them. Baggies are suggested to put jewelry in during class since students are not allowed to wear it during physical education class. • Students will be given a locker and lock in gym class. It is the responsibility of the student to lock his/her important possessions in their locker.

  15. “Specials” • Technology (one marking period) • Dramatic Arts (one marking period) • Spanish (one marking period) • Music Appreciation (one marking period) • Instrumental Music (for specific students) (IF they are taking Music it will be every other day and paired with Spanish or Techniology)

  17. Lunch (12:39-1:19) • Breakfast is important since the 6th grade begins lunch at 12:39. Breakfast $1.85 • Full lunch is $3.35. Extra portions are also available. Lunches can be pre-paid.(not good for students to have excess cash in school!) • I.O.U. is available if a student forgets a lunch. • Snacks are sold at the snack bar. ($.75- $1.50) • Students sit with their homeroom during lunch. • Bathrooms are open during lunch. • Online meal payment – support@myschoolbucks.com or 1-855-832-5226

  18. Phone Calls • Cell phones: If a cell phone is visible during school hours, it will be confiscated. Cell phones should be turned off and locked in the student’s locker. Cell phones will only be returned to a parent/ guardian. Consequences will be given. • If the camera is used – for example in the locker room it could become a police matter • Please communicate with your child before they leave for school in the morning. • Calling the MS to relay a message should only be done for emergencies.

  19. Dress Code Please refer to the student handbook for all the regulations of the Student Dress Code. Examples: Flip-flops are prohibited. No headbands, bandannas, chains and/or large jewelry are to be worn. Short shorts and tops are not permitted. No hats, sunglasses or outside jackets are to be worn in the building. Students are not permitted to wear clothing that is clearly sleepwear. Ripped jeans are prohibited. Clothing should fit appropriately.

  20. “I lost my…” • Lost and Found bins are located outside the cafeteria. • Students should not bring anything valuable to school. (I-pods, PSPs, cameras, jewelry, etc.) • Label everything with at least initials.

Early Pick-Up • If you are picking up your child early from school, please send them in with a note in the morning. • When you come to pick up your son or daughter, please have identification with you. • Students can only be picked up by people listed on the emergency card. • Must have the emergency card fill out correctly

BUSES • Busses are moving classrooms • Students need to demonstrate self-control • Disruptive/unsafe behavior will result in discipline – continued poor behavior may lead to loss of bus privileges • Students must ride their assigned bus/ Students can NEVER ride a different bus • Parents must write a note that needs to be approved by the administration for a bus student to be able to walk. • Students must get off the bus at their assigned stop - only • There is no food or drink allowed on the bus.

  24. Late buses • Used for after school help, after school meetings and detention. (Not for visiting the public library, Franks or Rita’s) • Students must have permission to ride the late buses. • Bus stops are different from the morning/afternoon stops. Please check the website for these stops. • Late bus runs will transport MS and HS students.

  25. Technology/ Information • Students receive user codes and passwords to use the school computers. • Student/Parent checklists will be paperless. All forms will be online. • Please check the district website and MS website for information. www.sayrevillek12.netwww.sayrevillemiddle.net

This meeting will provide important information about all sports programs, medical forms, coaches, and more, as well as taking the place of the Mandatory HIB/Hazing Parent training that the coach’s present every year.   29. Information for Sports Physical Forms • Sayreville Athletics – Medical Clearance Paperwork 2017 -2018 • Directions for completing the M.S. Clearance Packet • Who needs to complete the Middle School Clearance Packet? All student-athletes must complete the entire Clearance Packet for every sport they plan on playing/trying out for. • 1) Before completing this packet, you must go to the school website to read various informational and consent forms. To do this, go to www.sayrevillek12.net. Then click on “District Schools” and access the Sayreville War Memorial website. From there, go to “Athletics/Activities”, then “Athletics”, then “District Physical Forms”. • 2) Read all the information on this page, then click on each link under “Essential Informational and Consent Forms” and read each document carefully. Once everything has been read, you and your child can download and sign the Middle School Clearance Packet; it is a six page packet. • 3) Only the current Clearance Packet will be accepted (each page has 2017-2018 at the bottom). All forms must be completed and signed. Please keep all pages in the order they are printed. Make a copy of all forms for your records before handing them in. • 4) If you already have taken the Impact test for a prior sport in a prior season/year, you do not need to take it again. Only students who have never taken the Impact test should take it for this school year. • 5) During the school year, completed forms must be turned in directly to the School Nurse. All forms should be turned in at the same time. Incomplete forms will be returned to the student for completion by the parent or primary physician. During the summer break, forms should be returned to the Athletic Physical Collection Box at the High School Main Office.

  30. Information for Sports Physical Forms (Continued) 6) Fall sports physicals are due by August 1st. Students handing in physicals after this date may not be cleared in time to start the season. If you anticipate any delay in getting the paperwork in by those dates, you must inform your coach ASAP. Several forms cannot be dated more than 90 days before the start of the upcoming season. Be sure to stay within that time frame when filling out the paperwork. 7) Students that may need inhalers, epi-pen, glucagon or other medication during sports must have a current doctor’s order on file in the nurses’ office. Medication orders must be written on the district medication form available at the nurses’ office. It is strongly recommended that these students provide an extra inhaler, epi-pen, etc. to keep in their team’s medical kit if needed.

  31. Sayreville Athletics – Medical Clearance Paperwork Directions for completing the Physical Forms Who needs to complete the Pre-Participation Physical Evaluation Forms? Any student-athlete who does not have a physical examination on file, or whose physical examination will expire before the first day of their sport season, must complete the Physical Forms. If the physical examination will expire during the course of the season, the Physical Forms should be submitted once the new exam is performed. Physical examinations are only good for 364 days from the date of the exam. If your child has a physical on file that is good through the duration of the upcoming season, attach a copy of the yellow clearance letter that was mailed home to your clearance packet in lieu of the Physical Forms. If a new physical must be completed, the parent and physician must complete the Pre-Participation Physical Evaluation forms from the NJDOE. 1) To access the forms, go to www.sayrevillek12.net. Then click on “District Schools” and go to the Sayreville War Memorial website. From there, go to “Athletics/Activities”, then “Athletics”, then “District Physical Forms”. Click on the link for Pre-Participation Physical Evaluation Forms; this is a four page packet. 2) Page 1 (History Form) and Page 2 (Special Needs Supplemental History Form) of the packet must be completed by a parent. These forms need to be filled out before the physical exam and reviewed by the examining doctor. Both Page 1 and Page 2 must be completed and signed even if the student has no special needs. 3) Page 3 (Physical Examination Form) and Page 4 (Clearance form) of the packet must be completed by the primary doctor. Please be sure that everything has been filled out (e.g. height/weight, pulse, blood pressure, vision, etc.) or else the physical will be returned and have to be re-submitted. The form must include the date of the actual exam and the provider’s signature on both pages. 4) The healthcare provider you choose to complete the physical forms must sign the last page of the packet indicating that they have completed the “Student-Athlete Cardiac Assessment Professional Development Module”. Until directed otherwise by the NJ Department of Education, have them attach a copy of their completion certificate to the physical forms. After the physical packet is submitted, it will be brought to the school physician for his approval. Once the physical is approved, a yellow clearance letter will be mailed home indicating this. Keep this form for your records. Please note that this does not mean that the student is allowed to participate in their fall sport; they must also hand in a completed Middle School Clearance Packet. Once the student has handed in all of their district paperwork (i.e. the clearance packet) and the school doctor has reviewed their physical forms and approved them for activity, they will be fully cleared to participate in their sport and their coach will be notified. Make sure to check with your coaches well in advance of the start of the season regarding your clearance to avoid any problems on the first day of practice.

  33. Bullying “Harassment, intimidation or bullying” means any gesture, any written, verbal or physical act, or any electronic communication (Cyber), whether it be a single incident or a series of incidents, that is reasonably perceived as being motivated either by any actual or perceived characteristic, such as race, color, religion, ancestry, national origin, gender, sexual orientation, gender identity and expression, or a mental, physical or sensory disability, or by any other distinguishing characteristic, that takes place on school property, at any school-sponsored function, on a school bus, or off school grounds as provided for in (this statute), that substantially disrupts or interferes with the orderly operation of the school or the rights of other students and that:      a.     a reasonable person should know, under the circumstances, will have the effect of physically or emotionally harming a student or damaging the student's property, or placing a student in reasonable fear of physical or emotional harm to his person or damage to his property; or     b.     has the effect of insulting or demeaning any student or group of students; or c. creates a hostile educational environment for the student by interfering with a student’s education or by severely or pervasively causing physical or emotional harm to the student. Keep a watchful eye on negative things your children are writing and saying to others! (Please refer to the district website to view the policy) * Families do have the right to fill out an HIB report which will be investigated by both the VP as well as the HIB Specialist. N.J.S.A. 18A: 37-14 (2011).

  34. DISCIPLINE With your help we would like to prevent poor behavior before discipline is needed! Types of • Lunch Detention • Afterschool Detention 2:50 – 3:20 (Walkers/parent pick up may leave at 3:24. Bus students will stay with teacher and be picked up at 4:00) • Bus Suspension • In School Suspension (ISS) • Out of School Suspension (OSS) Remember – Some times good kids make bad decisions! The question we need to ask our student is - Are you going to repeat that bad decision?

  35. CYBER ISSUES We are not the Cyber police We will potentially deal with matters that have an effect on students at school Please know that the electronic devices parents are giving students are very powerful. These devices can help our children as well as harm them. Parents – Please closely monitor your child’s use/abuse of these tools.

  36. SMS PENNANT AWARDS • Star Student: Awarded monthly by teachers to students who have consistently exemplified a conscientious effort towards academic achievement and the practice of good character. • Most Improved Student: Awarded each marking periodby teachers to students who have shown improvement in academics and/or behavior. • Athlete: Awarded to students who participate in SMS athletics. (Soccer, Cross-Country, Field Hockey, Basketball, Wrestling, Cheerleading, Softball, Baseball) • Club Member: Awarded to students who participate in clubs and activities. (Chorus, Instrumental, Odyssey of the Mind (OM), Yearbook, Floor Hockey, Track Club, Tennis Club, TAG,Wise Times, Student Council, IMPACT/Community Service Club, School Musical, Step Squad, Game it Up!, Literary Magazine, etc.) • Perfect Attendance: Awarded to students who have perfect attendance each marking period. • I Made the Honor Roll at SMS: Awarded to students who received High Honor Roll (Straight A’s) or Honor Roll (A’s and B’s). Stickers will be awarded for the first 3 marking periods. • Random Act of Kindness: Awarded by Staff to a student who performs a self-less act or deed for a person or persons. (Returning: cell phone, money/wallet, jewelry, holding a door, picking-up books, etc.) Four stickers = Tee-shirt! • Leading by Example: Awarded by Staff to a student who shows leadership traits that separate him/her from others. Someone who acts as a role model; academically, socially and/or behaviorally. Someone who stands up for others who are being bullied. Three stickers = Tee-shirt! • School Community Award: Awarded by Staff to a student who goes above and beyond to contribute to the Sayreville Middle School community. (Beautification Day participants, Peer Leaders, students who assist teachers, special classes/ programs, etc.)

  38. The first day as a Sixth Grader • As you get off the buses, out of cars or approach the building as a walker, teachers will direct you where to gather on the patio. At approximately 8:05- 8:15 you will be given direction to enter the building through the gym entrance and go up to the gym to meet with your homeroom teacher. When we are all together, teachers will escort their students back outside and re-enter the building through the appropriate entrance. • (This is a Practice Run) • We will do this for the first couple of days.
