GENERAL FEES • Registration fee: $145.00 • Retail Natural Gas Supplier and Aggregator Financial Evaluation fee: $50.00 per evaluation • Retail Natural Gas Supplier Customer Information List fee: $150.00 per list • Governmental Aggregator Eligible Customer List fee (based on zip codes): $400.00 per list • Governmental Aggregator Eligible Customer List fee (includes best efforts verification of governmental boundaries): $1,200.00 per list • Returned check charge: $20.00 per check

  4. OTHER BILL PREPARATION REQUESTS • Request by a retail natural gas supplier or aggregator for a one-page duplicate bill: $0.26 per bill • Fee for providing Commission-mandated abandonment notices as bill messages: $0.3325 per bill

  5. BALANCING SERVICES • A monthly invoice is issued for daily overruns and underruns throughout each calendar month per the Gas Supply Aggregation/Customer Pooling Agreement, Rider EFBS (Sheet No. 50), or Rider FBS (Sheet No. 75) • A supplier who elects to receive service under Rider EFBS (Sheet No. 50) will manage a bank of natural gas on Duke Energy’s system

  6. LATE PAYMENT CHARGES • 1.5% of the unpaid balance at the due date, calculated monthly • Fee applies to all monthly invoices issued by Duke Energy Ohio, Inc. to suppliers

  7. APPLICABLE RATE SCHEDULES Reference rate schedules: • PUCO Gas No. 18, Sheet Nos. 44, 45, 50 (Rider EFBS) and 75 (Rider FBS)
