  4. SBAR: Purpose and Implementation Strategies Welcome and Introductions • This session is designed for the entire interdisciplinary team, including the: • Project champion and co-champion • DON, key RNs and LPNs • Medical director, primary care MDs, and NPs/PAs

  5. SBAR: Purpose and Implementation Strategies • Laurie Herndon is a Gerontological Nurse Practitioner at Fallon Clinic in Worcester MA. She received her Master of Science in Nursing at Simmons College. She is currently the Director for Clinical Quality at Massachusetts Senior Care Foundation. • Laurie has worked as a project consultant for a number of New England Quality Improvement Organizations on topics including Depression, Falls, and Medication Management.  • She holds a clinical faculty position at the University of Massachusetts Graduate School of Nursing in Worcester where she served as the project coordinator for CoGEMS: Comprehensive Geriatric Education and Mentoring Across Settings laurieherndon@yahoo.com

  24. Homework (1) SBAR: Purpose and Implementation Strategies • Select two implementation strategies from suggestions provided (One strategy must include getting started with the SBAR on at least one unit) • Implement the SBAR on at least one unit with at least one nurse for the next two weeks

Homework (2) SBAR: Purpose and Implementation Strategies • Prior to the next call, ask staff who have been using the SBAR for the following feedback: • How has using the SBAR influenced your conversations with MDs/NPs/PAs when you are calling them about a change in a resident condition • Do you have any questions or suggestions about the SBAR? • For the next call, be prepared to take 1-2 minutes to share: • Which two implementation strategies you selected • Feedback from the staff
