Reminder… • When enrolling a dependent for the first time or making a qualified change during the year or during annual enrollment you will need to submit appropriate documentation to support proof of dependency. • Definition of Eligible Dependent: • A lawful spouse • Your unmarried children who • Are less than 26 years of age; • Do not have access to their own insurance through their employer; • Over 26 years old and primarily supported by you and incapable of self-sustaining employment by reason of mental or physical handicap.

  17. Flexible Spending Accounts You must re-enroll each year to participate!!! HC FSA Limit reduced to $2,500 • You may put between $100 and $2,500in your Health Care FSA • You may put between $100 and $5,000 in your Dependent Care FSA • Child Day Care • Elder Day Care • Not Health Care If you currently participate in the FSA plan, do not destroy your BennyCard, as it will be reloaded for the 2013 Plan year with your new election amount. New participants will receive their BennyCards in the mail prior to January 1, 2013.

Life and AD&D Insurance Basic Life / Accidental Death & Dismemberment Insurance* (company paid) • 2X salary up to $500,000 Optional Life Insurance*(employee paid) • Employee • 1X, 2X, or 3X salary up to $1 million • EOI required for any changes or additions to coverage • Spouse (employee must elect at least 1X salary in optional) • Up to 100% of employee supplemental life benefit up to $250,000 • EOI required for any changes or additions to coverage • Children (must elect at least 1X salary in optional) • $5,000 or $10,000 • $0.40 or $0.80 per month * The benefit reduces 33% at age 65 and 50% at age 70. Disability Insurance Short-term Disability (employee paid) • 14-day elimination period • 60% of salary up to $1,000 per week (minimum = $25/week) • After-tax premiums = tax-free benefit • Benefit paid for 24 weeks (168 days) after elimination period • Seamless transition from short- to long-term disability • EOI required for any new elections Long-term Disability (company paid) • 60% of salary up to $11,000 per month (minimum = $100/mo.) • 180-day elimination period • Benefits received are taxable

Pre-Paid Legal (Hyatt Legal) • Plan gives you and your family access to legal services • Attorney fees fully paid by the plan when you use an in-network attorney • No limits on the number of times you may use the plan, and no dollar limits on use of an in-network plan attorney for the following services: • • Purchase, sale or refinancing of your primary residence • • Trusts, wills/codicils, living wills and powers of attorney • • Document preparation • • Immigration assistance • • Identity theft and debt collection defense • • Traffic ticket defense (no DUI) • • Elder law matters • • Uncontested adoption or guardianship • • Defense of civil lawsuits • • Eviction and tenant problems (primary residence tenant only)

  23. Pre-Paid Legal (Hyatt Legal) Enroll/waive pre-paid legal the same as other benefits. All enrollments are for the entire calendar year – no changes can be made during the year 2013 rate is unchanged at $26.65 per month ($13.48 per pay)

  25. 401(k) Deferral Limits for 2013 • The IRS will increase the maximum amount you may defer into your plan in 2013 to $17,500 (incr. from $17,000 for 2012) • As in 2012, employees 50 yrs of age or older in 2013 may defer an additional $5,500 in “catch-up” contributions (over-and-above the normal deferral limit) • The IRS has increased the compensation limit for 2013 to $255,000 (incr. from $250,000 for 2012) All 401(k) plan changes must be made directly into Milliman’s system at www.millimanbenefits.com no later than 2:59 PM CDT on 12/28/2012 in order to be effective during the first quarter of 2013. Your catch-up contribution election will automatically roll-over for plan year 2013, unless you go out to the Milliman web site and dis-enroll yourself.

  27. 2013 Benefit Election Confirmation Statements Statements will be mailed to your home address the week of December 5th. You should… • review your statement to determine there are no data entry errors on our part, • correct any data entry errors by making the pertinent changes on the statement, and • sign and date the statement and return it to the benefits office in Dallas The Benefits office in Dallas must receive your corrected statement by5:00 p.m. (CST) on Thursday, December 20, 2012. If everything is correct, then no further action is required on your part.
