Phase I – Capacity Building and Education • Establish organizational infrastructure • Partnerships • Including local communities, primary and secondary prevention facilities, diagnosis and treatment facilities, government and non-government programs • Participation in community-based primary and secondary prevention activities • Increase community participation in smoking cessation and early detection activities

  7. Preliminary First Year Program Evaluation Findings • 399 People work on the CNP staffs • 171 Clinical partnerships created with screening clinics, hospitals • 641 Community partnerships, local/state government, non-profit organization partnerships • 898 Educational activities during the first year

Phase II – Disparities Research and Training • Address spectrum of research issues necessary to reduce disparities in the community • Develop community-based interventions • Assessing policies on reducing disparities • Train researchers from minority/underserved populations in disparities research • Develop pilot projects to reduce cancer disparities, including assessment of health policy

  9. Phase III – Establish Credibility and Sustainability • Reduce cancer disparities in the community • Obtain non-CRCHD funding for continuing Community Network activities • Inform policymakers of community-based research that reduces cancer disparities
