  2. Funding and Staffing • Funding from the CDC • Budget: $274,000 • FTEs: 1 FTE health program associate .5 FTE epi

  3. Program Objectives • Increase the rate of: eye exams foot exams A1c testing Flu shots *Address health disparities *Wellness: pre-diabetes * Maintain a diabetes surveillance system

  4. Current workplan • Conduct professional education • Institute worksite wellness at state agencies • Work with Hill CHC on pre-diabetes • Assist CHC in the dm collaborative • Work with partners to plan large scale diabetes awareness events….continued

  5. Workplan continued • Increase the number of NCQA recognized providers (by 10) • Promote the diabetes standards of care to PCPs • Plan, implement and evaluate a patient incentive program ….continued

  6. Workplan continued • Promote flu shots through pharmacy and paycheck stuffers • Create and distribute a statewide diabetes newsletter

  8. Workplan continued • Develop a statewide diabetes data and surveillance workgroup • Provide data analysis for hospital discharge data on diabetes-related diagnoses, BRFSS and other sources
