FACILITY SECURITY PLAN (FSP) REVIEW PROCESS • NOTES: • Stage III includes; review of the assessment report, and any carry over items forwarded by the National FSP Review Center • One regulation review topic other than failure to submit FSA Report or CG-6025 is incomplete and proceeds to Tiger Team Stage II Review • No FSA report or 6025 submitted with plan or two or more of the other regulation review topic are incomplete, then the plan will be returned disapproved COTP forwards, or Facility Submits FSP to National FSP Review Center Stage III Final Review And Approval On-Site Validation By COTP Coast Guard Logs FSP Into MISLE Final COTP Approval (Note 1) Disapproved Notification of Receipt to Facility (Electronic or Hardcopy) Submitted to B&V For Review Annual Maintenance Notification & Return of FSP To COTP Disapproved (Note 3) Stage I Review Incomplete (Note 2) Complete Return for Resubmission (30 Days Max.)
