  2. STEP 1. Go to http://www.cms.gov/mcd/search.asp?clickon=search This is the CMS search area for LCD’s and NCD’s (Local Coverage Determinations). Under type of document select LOCAL COVERAGE DOCUMENTS

  3. STEP 2. Pick your Geographic area from the drop down list or select All States.

  4. STEP 3. Type in a keyword: in this case use HYPERBARIC and for the CPT/HCPCS code use C1300 and hit SEARCH.

  5. SEARCH RESULTS. Your search results will show up. In this case I searched all states so all the Hyperbaric LCD’s for all the states showed up on my query. Refine your search by just selecting one state.
