What we were looking for • Consideration about THIS child must include a discussion about the potential problems of Down’s Syndrome and VSD with respect to all modalities of treatment. • Some general comments about how these 2 diagnoses would affect your assessment of the child • A good discussion of options • A conclusion or statement about what YOU would do and why you would do it.

  3. What we got was • Good answers about the pros and cons of managing the bead in ANY 4 year old • A discussion without a conclusion, i.e. at the end of many papers I had no idea what you would do as an emergency physician if this child came to your ED tomorrow

  4. A good answer would have • Discussion of instruments/technique to use • Discussion about levels of sedation in the context of Downs and VSD (as well as those that pertain to a 4 year old) • A logical and readable layout • A final statement about what you would do and why
