  2. การติดตามวัดผล ปี 2555 Monitor ในส่วนที่ให้บริการตามกลุ่มเป้าหมายที่กำหนดเป็นหลัก คือ กลุ่มประชาชนทุกสิทธิที่มีข้อบ่งชี้ ดังนี้ ผู้ป่วยด้วยโรคเรื้อรัง 7 โรค(ปอดอุดกั้นเรื้อรัง หอบหืด หัวใจ ไตวาย หลอดเลือดสมอง ผู้ป่วยมะเร็งที่อยู่ระหว่างการได้รับเคมีบำบัด และเบาหวาน) ผู้สูงอายุ> 65 ปี ขึ้นไปทุกคน
