DA / RU Examen sous valvessutures Hémorragie en salle de travail t0 Noter l’Heure: t0 Mettre le monitorage Demander de l’aide Oxygène Vidange vésicale Oxytocine 10-20 UI perfusées massage utérin Vérif carte groupe Remplissage: Cristalloïdes, colloïdes ± Ephédrine Vérif VPP ± 2èVPP ± NFS-hémostase ± dispo du sang ±Hémocue t30 Maintenir PAM 60-80 mmHg 2èVPP+Hémocue Sulprostone : 500 µg (1 amp) en 1H (maxi) Sonde urinaire Diurèse / h + Bilan bio complet avec vasopresseurs si besoin: noradrénaline: 0,5mg/h à adapter CG : Hb ≈ 8-10 g/dL Antibioprophylaxie PFC:CG = 1:1 Plaquettes si <50-100000 + Fibri si < 1,5-2g Réchauffement KT artériel ? KTC veineux ? t60 Ac. tranéxamique? Ligatures artérielles Embolisation rFVIIa Hystérectomie (min) (d’après les RPC de déc. 2004 et adapté d’une diapo du Réseau de Montpellier)
