  4. The Application – Part 2 • LEGAL INFORMATION • Citizenship • Convicted of any Crimes, Felonies, or use any other name as a nickname or alias? • Military Service • Proof of Age if Necessary (Birth Certificate, Drivers License)

  5. The Application – Part 3 • EDUCATION HISTORY • Schools Attended or any Special Training (Include Military) • Beginning and Ending Dates of Each Level of Education • Expected date of Graduation (May 20--) • What degree did you earn? • Highest level of education

  6. The Application – Part 4 • EMPLOYMENT HISTORY • Employer/Company Name • Address • Phone # • Name of Supervisor • Position Held • Dates of employment • Salary (sometimes) • Reason for Leaving

Use black or blue ink, or even better, erasable pen. • Certain before you fill in the blanks. • Prepared with all of the information you need. Bring note cards with all the information needed. • Complete and answer every question. If it does not apply to you, write “does not apply” instead of leaving it blank. • Correct with spelling, grammar and punctuation. • Accurate with all of your information.
