  3. 1. Biodata & Identitas apa bila ada kesalahan nama, tempat dan tanggal lahir, alamat, jenis kelamin dan status perkawinan

  4. Bentuk biodata & identitas

  5. 2. Riwayat & Prestasi Apa bila ada perubahan data seperti riwayat PNS, riwayat pendidikan, fungsi dan jabatan, riwayat mengajar, serta sertifikasi dan diklat.

  7. 3. Pengajuan Sekolah Induk Apa bila PTK akan berpindah sekolah induk, seperti dari sekoah ke sekolah lain dalam satu kab/kota, dari sekolah kesekolah lain lintas kab/kota, serta dari sekolah ke madrasah (dinas ke kemenag )

  8. Yang harus dilakukan PTK adalah :1. klik cetak surat ajuan2. masukkan nama provinsi dan kabupaten3. kata kunci dengan memasukkan NPSN sekolah induk yang baru4. cetak surat ajuan dan bawa kedinas pendidikan.
