Choose the right target: Root cause analysis Choose appropriate goals-objectives-outcome measures Choose the right interventions/actions: Evidence-based practices

Techniques • Appreciation • 5 Whys • Drill down • Cause-and-effect diagram
Steps for Root Cause Analysis • Define problem • Identify possible causal factors (broad categories) • Identify real causes behind those factors • Identify and implement solutions • We will cover this later
Define Problem • Look at your CHA • Use data when possible • Stick to the facts – no judgment • Be specific
Example • Priority area: OBESITY

Identify Possible Causal Factors • What sequence of events leads to the problem? • What conditions allow the problem to occur? • What other problems surround the occurrence of the central problem?
Let’s do it! (15 minutes) • Brainstorm for a few minutes • Write possible causes on Post-It Notes • Place Post-It Notes on chart • Identify major categories of causes (4-6) and move Post-It Notes to categories • Arrange causes logically (cause and effect) • Analyze the diagram ISSUE: 30 percent of residents in our county don’t reach the recommended daily level of physical activity
