  2. ב- cochrane reviews נבדקו הטיפולים הבאים: • טיפול בסטרואידים • Nebulized epinephrine • אינהלציה של אוויר עם אדי מים • אינהלציה של גז HELIOX

  3. טיפול בסטרואידים (דקסהמטזון ובודזוניד) • מעודכן ליולי 2010 - הוכללו 38 מחקרים n=4299 • נראה שיפור ב WESTLEY SCORE לאחר 6 שעות (mean diff. -1.2), וב 12 שעות (mean diff. -1.9), לאחר 24 שעות לא נמצא הבדל • פחות אשפוזים וביקורים חוזרים במיון (RR 0.5) • ירידה של 12 שעות בממוצע של שהיה במוקד רפואי

  4. Nebulized epinephrine • סוקרו 8 מחקרים (225 משתתפים) • ניכר שיפור משמעותי סטטיסטית ב CROUP SCORE 30 דק' לאחר המתן • השיפור לא היה משמעותי שעתיים ושש שעות מאוחר יותר • הביא לקיצור משמעותי במשך האשפוז (32 שעות ב RCT אחד) • לא ניכר יתרון של racemic epinephrine או Leאו IPPB

  5. אינהלציה של אוויר עם אדי מים • הוכללו 3 מחקרים מחדרי מיון שכללו 135 ילדים עם CROUP קל-בינוני • לאחר 20 ו 60 דקות היה שיפור קל בקב' ההתערבות (weighted standardised mean difference of -0.14 ) • דרושים מחקרים נוספים

  6. Exposure to cold air • לא נחקר באופן שיטתי – מתוך UPTODATE "Exposure to cold night air also may lessen symptoms of mild croup”

  7. אינהלציה עם גז HELIOX • הוכללו 2 מחקרי RCT עם 22 ילדים בזרוע ההתערבות ו 22 ילדים בקב' בקרה • בשני הניסויים לא השוו HELIOX לפלסבו (חמצן 30% +לחות או racaemic epinephrine ) • לא היה הבדל משמעותי ב CROUP SCORE בין הקב' השונות • דרושים מחקרים נוספים

  8. Westley croup score • Level of consciousness: Normal, including sleep = 0; disoriented = 5 • Cyanosis: None = 0; with agitation = 4; at rest = 5 • Stridor: None = 0; with agitation = 1; at rest = 2 • Air entry: Normal = 0; decreased = 1; markedly decreased = 2 • Retractions: None = 0; mild = 1; moderate = 2; severe = 3 • The total score ranges from 0 to 17. • Mild croup is defined by a Westley croup score of ≤2. • Moderate croup is defined by a Westley croup score of 3 to 7. • Severe croup is defined by a Westley croup score of ≥8

  9. Tx. Of Mild Croup • symptomatically with humidity, fever reduction, and oral fluids • Exposure to cold night air • אם מגיעים למרפאה ניתן לטפל במנה חד פעמית של דקסהמטזון P.O (0.6 מ"ג/ק"ג)

  10. Tx of MODERATE TO SEVERE CROUP •  Mod. Croup -> ED or office (intubation) • Severe croup -> ED • Supportive care — humidified air or humidified O2 if O2 sat. <92% or respiratory distress • Dexamethasone (0.6 mg/kg) single dose • Nebulized Epinephrine (Racemic epinephrine: 0.05 mL/kg per dose  L- epinephrine: 0.5 mL/kg) • Decrease anxiety • Supervision -> Intubation
