2. 2 Army Periodic Medical Exams Active Component  Every 5 years beginning at age 30 (Army requirement [Army regulation 40-501] not Title 10 requirement)
Reserve Component  Every 5 years from time of enlistment/appointment IAW 10 USC 10206 
3. 3 Scope of Periodic Physical Exam Hands on Physical Exam 
Plus:
Audiometry
Vision Screen
Males: Testicular Exam 
Age 40 +, add:
 ECG
Males: Prostate exam 
Intraocular pressure  
4. 4 Scope,  Cont. BP, Temperature, Height, Weight
Labs: 
Urinalysis for sugar/protein, HCT/ HGB, Cholesterol (most labs do lipid profile), HIV
Age 40 or over add: Stool for Occult Blood, Fasting lipid profile, PSA test (males), FBS. 
5. 5 Female Exams Female soldiers on active duty: Annual PAP/Pelvic (includes screen for chlamydia for those under age 25)
Annual Breast Exam
Active Duty Female: Mammography  ages 40, 42, 44, 46, 48, 50 then annually (more often if clinically indicated). 
6. 6 Other Female Exams, cont. Reserve Component Females: Same requirements as active duty if serving on an active duty tour for 1 or more years
If not on active duty - PAP/Pelvic/Breast Exam required as part of 5 year physical.  No mandatory requirement for annual screen or for mammogram
