MAPP Assessments Overview • Four MAPP Assessments • Community Themes and Strengths • “What is important to our community?” • Local Public Health System • “How are the Essential Services being provided?” • Community Health Status • “How healthy is are our residents?” • Forces of Change • What is occurring or might occur that affects the health of our community or LPHS?”

Community Health Status Assessment • METHOD: • Collect, analyze and review public health data • Describe population health status • Identify health issues by assessing • Changes over time • Differences among groups • USES: • Develop priorities • Plan actions to improve health outcome • Assists public health and emergency mgmt in readiness/preparedness plans

  5. Step 1 – Prepare for the Assessment • Establish subcommittee • Planning considerations • Include individuals with some experience or expertise in data analysis and interpretation • Not ad hoc committee, work in monitoring will continue

Step 2 – Review of Data • Identify locally relevant indicators • Collect and analyze data • Use data from previous assessments • Access data from partner agencies • Review trend data • Examine data over time • Note direction (increase/decrease) • Comparison data • Goals/Benchmarks – HP2020 • Place – State, County, Region • Person – Age, Gender, Race,

  9. Step 2: Review of Data: State/Local Sources • State/local health department websites • Contact local agencies that collect health status and services data • e.g. local health department, health and social service agencies

  10. Step 3 – Organize and Present Findings in Community Health Profile Report • Elements: • Narrative, data tables, comparison graphs, trend lines, maps, etc. • Priority issues identified by community • Uses • Discussion • Strategizing • Action planning • Advocacy

Benefits – Community Health Profile Report • Highlight relationship between health status and determinants of health • Define comparisons • Motivate leaders to address issues • Serve as reference documents • Assist in allocation of resources, strategic planning • Resource for grant application processes • Unite communities around issues

Step 4 – Present the Data: Identify Indicators for Improvement, Maintenance of Effort, Additional Data • Where are we doing well? • How did we get there? • How do we maintain our success? • Where do we need to improve • Reduce disparities • Reduce risk factors and social determinants • Where are data/indicators missing • Why are data unavailable? • Can we develop reliable indicators?
