At Medway Country Manor, we've taken significant steps to improve the testing and diagnosis of urinary tract infections (UTIs). Our goals include treating only UTIs with specific symptoms and reducing unnecessary antibiotic usage. Over the past year, our nurse practitioners received targeted training, enabling them to refrain from treating positive cultures without symptoms. We educated staff on appropriate antibiotic usage while being cautious of C. difficile risks. This initiative required overcoming challenges, but our NP leadership and ongoing education have resulted in improved compliance and patient comfort.

How we are improving testing and diagnosis of UTIs atMedway Country Manor Marjorie Kenney, RN Director of Nursing
Goals • Treat only UTIs with specific symptoms • Reduce unnecessary antibiotic use
What we did Began work about a year ago • NPs attended training • Not treating positive cultures when there were no symptoms • Staff education during the C. difficile project on appropriate antibiotic use and UTIs • Cut down on some of the flouroquinalones because of the links to Cdiff Education • Formal in-service sessions • “Real time” education; followed up, asking questions looking at cultures
What seemed to matter? Nurse Practitioner leadership • Talked with families • Educated staff about overuse of abx
Challenges and Strategies Some staff resistance • Association of mental status change with a UTI • A year in, people are more on board • Nurse practitioners lead by not ordering antibiotics; staff notice that people didn’t get sicker and were more comfortable with the change
