  1. …as a woman participant: Must be: African American woman 21 years of age or older Pre-hypertensive, hypertensive and/or overweight or obese Have a family member who is willing to participate, particularly a child ages 9-17 Must be willing to: Attend a screening day, 1-2 workshops, health-smart seminar Participate in a vegetable cook-off and at least 3 sessions of physical exercise per week Utilize Health-Smart Church Center Complete health data questionnaire Sign Informed Consent Form, consenting to have weight, blood pressure, and BMI checked …as a Church Leader Health Empowerment Coach: Can be: Any member of the church who has the time and commitment to help lead their church members to live healthier lives! Must be willing to: Attend a 3 hour workshop about health-smart behaviors Serve as trained Health Empowerment Coaches Serve as healthy life style role models to motivate church members6 Coordinate monthly exercise sessions Judge in a Healthy Vegetable Cook-Off Complete health data questionnaires Church Health Program **For more information, please contact Ms. Priscilla Jenkins after the church service.
