  4. 2013 IHI HEN Goals • 100% of hospitals with OB services have a hard stop policy in place or a sustained EED rate of less than 3% • 100% submission of EED measure into CDS • Share a HEN Culture Story • Submit one progress report each month • Participation in HEN Week/Physician Leadership Conference/SWW

  6. Congratulations to Our Lady of the Resurrection Medical Center, Pekin Hospital, Presence St. Joseph Medical Center, Red Bud Regional Hospital and Rockford Memorial Hospital for meeting 100% of their applicable goals so far – there is still plenty of time for the rest of you to get caught up!

Level of Participation Report • We have used the data submitted into the CDS to rank your progress to date according to the Z-4 criteria. • We need your help assessing whether you are 4’s or 5’s. • Please use the guidelines in your packets to tell us whether you consider yourself to be a 4 or 5 in all areas where you are currently ranked a 4. This report will be updated on a monthly basis, so if you don’t feel you could be ranked a “5” right now – don’t worry! • Please note – if you are not submitting data for EED, you cannot be ranked higher than a 1! Just another reason why it is very important to get your EED data into the CDS!

  10. Patient and Family Engagement & Leadership Also included in the Level of Participation Report is a scoring system for Patient and Family Engagement and Leadership. Please also complete this worksheet so we can update your scores from the last time you were polled on the webinar in February.
