  1. M/s Rozer Plant, Raipur Faulty equipments. Man Basket was being used by Ace Hydra crane without limit switch. Status- (Done) Stop use of Ace crane for Man basket and provide another hydra with auto hook compensation system for man basket lifting.

  2. M/s Rozer Plant, Raipur Lack of Access. Due to short height ladder worker used to detach there full body harness during coming on rafter from ladder. Status- Asked to provide full length ladder with fall arrester.

  3. M/s Rozer Plant, Raipur Procedure violation. Eight no. column were erected without any guide rope also there were only one girt provided in between columns. Status- No. of Connecting girts to be increased..
