  3. Team OZL • gestart: 1 april 2006, na ≥ 2 jaar voorbereiding • in 2007: 38 consulten • tot nu toe: 15 consulten Invitational Conference IKL 24 juni 2008

  4. Samenstelling team • 2 huisartsen • 1 verpleegkundige ziekenhuis • 1 verpleegkundige thuiszorg • 1 verpleeghuisarts (doet geen bedsides thuis) knelpunt: kwetsbaarheid team Invitational Conference IKL 24 juni 2008

  5. Deskundigheid team • werkzaam in de palliatieve zorg • scholing • achterwacht regionaal team • wekelijkse afvaardiging naar overleg regionaal team (vlgs. rooster) • 6 wekelijkse casuïstiekbespreking eigen team Invitational Conference IKL 24 juni 2008

  6. Bereikbaarheid team • maandag, woensdag en vrijdag • volgens rooster • via “Maastricht” • via hospice • via consulent rechtstreeks Invitational Conference IKL 24 juni 2008

  7. Financiering team • tijd • vrijgeroosterd van patiëntgebonden taken • geld • consulturen door IKL vergoedt knelpunt: niet structureel Invitational Conference IKL 24 juni 2008
