Terminology Changes • “Other Service Efforts” is now called “Other Funding Sources” • “Description of Target Population” is now called “Description of Population to be Served”
User-Friendly Modifications • Added an unlimited text field to allow you to elaborate on the “Description of Population to be Served” • Added additional fields for listing service providers – Homemaker, Personal Care, Transportation • Added additional fields to the Ombudsman Objectives • Added additional fields to the Delivery Sites for Group Services
Unlimited Text Field Added for “Description of Population to be Served” • Throughout the plan we added an unlimited text field below “Description of Population to be Served” • Allows you to elaborate on the population to reflect your agency’s specific activities
Additional Fields to List Service Providers • Additional fields were added to list service providers for programs that usually have more than 10 service providers: • Homemaker (p. 22, 70) • Personal Care (p. 24, 72) • Transportation (p. 88) • There are now 15 fields instead of 10 fields • Will eliminate having to send the template back to VDA to add the fields • Saves Time
Additional fields for Ombudsman Objectives • Additional fields were added to the Ombudsman Objectives (see p. 92) • There are now 10 fields instead of 5 fields
Additional fields for Delivery Sites for Group Services • Additional fields were added to the Delivery Sites for Group Services to allow you to list more locations (see p.55) • There are now 25 fields instead of 17 fields
“No” Check box Removed from Waiver Pages • The “No” check box was removed from all Waiver Pages throughout the plan • Check “Yes” check box if applicable • Leave “Yes” check box blank if not applicable
