  6. Does this person have serious trouble seeing or are they blind?

  7. If no mention of use of devises, what about for example… • … long-sighted, astigmatical, cataract, glaucoma …. will they identify with the definition?

  8. Does this person have serious trouble hearing or are they deaf?

  9. Does this person usually have serious problems remembering or concentrating?

  10. We can’t get it all, but we have to be aware of and report what we are not getting
