  3. WHAT BENEFITS DO WE OFFER • Medical • Dental • Stand Alone Dental • Standard Life • Optional Life Plans (1 & 2) • Dependent Life Plans (1 – 4 options) • Flexible Spending (Health and Dependent Care) • Long Term Care • Pension Plan • 401k Plan • EAP – All employees to include flex

What Benefits require paperwork? • Medical – Must have waiver or enrollment (499 and provider form) • Dental – Must have a waiver or enrollment (499 and provider form) • Stand Alone Dental- must have for enrollment (499 and provider form) • Standard Life – Must have a waiver or enrollment (499 form only) • Optional Life Plans (1 & 2) must have for enrollment (499 form only) • Dependent Life Plans (1 – 4 options) – must have for enrollment (499 form only) • Flexible Spending (Health and Dependent Care) – must have for enrollment • Long Term Care – must send enrollment to CNA • Pension Plan - must have a waiver or enrollment • 401k Plan – automatic waive, must submit quick enroll for new hires only, all others must contact fidelity directly.

  5. THE 499 FORM WHY IT IS SOOO IMPORTANT

Form MCCS 499 is the employee’s benefit history and must be completed each and every time the employee has a benefit change/update. • MCCS 499 is to used if there is any change the following benefits: • Medical Plan – enroll/update/terminate • Dental Plan – enroll/update/terminate • Life (Beneficiary update)/terminate • Dependent Life changes (adds/deletions) • When updating the form be sure every plan and dependent currently enrolled are listed on the form.

  7. Failure to complete the 499 forms could result in one or more of the following: • Employee and/or Dependents coverage being cancelled • A Beneficiary not being listed for Life Insurance • Unable to verify coverage for retiring/terminating employees • The 499 form will be used as verification for continuation of benefits when an employee retires.
