  2. Clients Focus: • What are you going to do to maintain your housing? • Complete up to 3 monthly goals that are related to their housing sustainability. (Ex. Job, budgeting, pay bills on time, etc.)

  3. Case Manager's Focus: • Reinforcing that assistance is limited and it for a short period of time • Preparing the client for when the case manager will no longer be working with them • All households compositions are different so they are evaluated on a case to case basis depending on the clients need • Providing clients with resources that they need to maintain their housing. (Ex. List of pantry's, utility assistance, CT Works, etc.)

  4. Success looks different for each family.
