The nation is aging. • In 2030, nearly 1 in 5 US residents will be 65 or older The US health care system is not equipped to deal with a rapidly growing population of older adults with multiple health conditions. • 20% of Medicare beneficiaries with 5+ chronic conditions account for more than 2/3 of Medicare spending Current system failure takes heaviest toll on those most vulnerable – race, ethnicity, gender, language and socioeconomic status.

Absence of Patient-Centered Care Comes at a Cost Inappropriately prescribed medications 37 visits 14 different doctors 50 prescriptions Duplicate Tests & Procedures No one to find & arrange non-medical services Too Frequently Hospitalized 1 in 10 readmitted within 15 days & 1 in 6 back in hospital within 30 days Little or no info upon discharge
