  11. Designs are only limited to one imagination and free time.

  12. Question 1: How personalized? • Generic with blank for patient name and/or procedure: • Can be printed out just like business cards for really cheap • Can be easily filled out by anyone in clinic or even right in the OR • More professional with typed name of patient and procedure: • Looks much more professional than fill in the blank cards • Patients more likely to hang onto these or request a replacement • More tricky to print • May requiring special printer or basic print-shop know-how • Personalized with patient’s actual x-ray: • Patient much more likely to keep this unique and very personalized card • Much more difficult to produce • Requires special printer, software and competent/dedicated staff • Difficult to get to patients immediately post op

  13. Anyone can fill in the blanks on a generic surgery cards. These serve the purpose of having something for patients that want something to take with them to the airport. These can be preprinted in mass and are economical. In case of emergency contact: UWMC Bone and Joint Surgery Center 4245 Roosevelt Way NE, Seattle, WA 98105 Office: 206-598-4288 Paging Operator: 206-598-6190 Surgery information available at www.UWOrtho.com Shoulder Replacement Patient: Surgery Date: Patient requires antibiotics prior to any dental or surgical procedure. Implant may activate metal detectors Front Back

  14. Custom made cards with the patient’s name and procedure are easy enough to set up and can be done by most staff members with minimal input. These can be preprinted prior to surgery and given to patient immediately post op. Quite dandy. Patient Identification Card Name: John Doe Medical Number: U1234567 DOB: 23 November 1955 Surgery: Total Shoulder Arthroplasty Date of Surgery: 11 June 2008 Surgeon: Winston J Warme, MD In case of emergency contact: Winston J Warme, MD UWMC Bone and Joint Surgery Center 4245 Roosevelt Way NE, Seattle, WA 98105 Office: 206-598-4288 Paging Operator: 206-598-6190 Surgery information available at www.UWOrtho.com Total Shoulder Replacement This patient has a total shoulder replacement. This replacement may activate metal detection devices. Patient requires antibiotics prior to any dental or surgical procedure. John Doe U1234567 Front Back

  15. Custom made cards with the patient’s personal x-ray will require some basic graphics software and are a little more tricky to setup. These would have to be printed after surgery. Done right, these would be very special little cards, but would be too much trouble to produce for some services. Patient Name: John Doe Medical Number: U1234567 DOB: 23 November 1955 Surgery: Total Shoulder Arthroplasty Date of Surgery: 11 June 2008 Surgeon: Winston J Warme, MD Implant may activate metal detectors Antibiotics required prior to any dental or surgical procedure Present card prior to MRI examination In case of emergency contact: Winston J Warme, MD UWMC Bone and Joint Surgery Center 4245 Roosevelt Way NE, Seattle, WA 98105 Office: 206-598-4288 Paging Operator: 206-598-6190 Surgery information available at www.UWOrtho.com Total Shoulder Joint Replacement John Doe U1234567 DOB: 07/27/1945 Implant may activate metal detectors John’s Shoulder Front Back

  16. Question 2: How professional appearing? • Generic card with blank for patient name and/or procedure: • We already have these • They look cheesy, but cost only as much as a box of business cards • Easy to fill out, even by orthopedic surgeon • Double sided printed on card stock: • Can be customized as much as you like – much more professional appearing • Requires special printer and/or special training • Print and fold lamination: • Lasts longer than un-laminated card stock • Need laminator • Plastic ID card: • Really nice • Needs special printer and software

  17. Question 3: How much do you want to spend? Generic cards are cheap (we already have them) A laser color printer is pricey – but we already need one for other projects A card laminator has an initial setup cost and will require card pouches (which aren’t that expensive) A plastic card printer may be very pricey considering maintenance and operating costs and is only worth the expense if we are planning on producing a lot of cards over the next many years and want a very professional end product Outsourcing is going to be extremely expensive in the long run at several dollars a card

  18. Preprinted generic cards – fill in names if you like • Most economical ID card option. • Very easy to fill out. • Least professional looking option. 19. Printing single sided grayscale cards on cardstock with our printers • Very economical option – only need card stock or perforated business card sheets • Requires basic/advanced MS Office setup skills – which not all MAs and RNs have Note: Adding x-rays is really cool, but requires a good amount of Centricity/PACs know-how, as well as some advanced MS Office expertise. Photo editing software would also be useful.
